Application Description
This form will be used to ensure that when an employee returns to work that they have the required equipment, user accounts, and system access to perform their job duties.
When recalled employees are approved by the RBC, this form should be filled out by the Division Director and returned to the IT department electronically. After IT requests are complete, the form will be forwarded to the other required Divisions.
This form should be submitted 5 days prior to the employee’s date of return to ensure they have the required access to perform their job duties.
